Request Reports

All ETV DWS Center Verification Reports are posted on the NSF International website as PDF files. You may download and print out the reports at no charge. By submitting the following form, you may request additional copies of the Verification Reports if you are unable to use the posted reports on the website. Verification Report Appendices are not posted to the NSF International website and can be requested by contacting Bruce Bartley at bartley@nsf.org.

  1. Electronic Format: A PDF copy of each requested verification statement and report can be e-mailed to you free of charge.
  2. CD-ROM: A CD-ROM can be mailed to you containing each requested report for the following fee - the first CD-ROM costs $100.00 (USD) with additional copies of the CD-ROM costing $50.00 (USD).
  3. Hardcopy Format: A hardcopy of each requested report can be mailed to you for the following fee - the first copy of each report costs $100.00 (USD) with additional copies of the same report costing $50.00 (USD). You may request a hardcopy of each verification statement free of charge.
