I certify that the list of NSF Registered products for my Company as listed on the Nonfood Compounds website as of the date of this statement, represents a true and accurate list of product(s) that are NSF Registered.

I further certify that no changes have been made to said products; that is, (changes in registered company name, product name, formulation or product label) as outlined in the Definition of Registration Amendments since the NSF Registration Letter was issued.

I have indicated below any registration amendment(s) that are needed to bring said product registration(s) into compliance.

I understand that making changes to a product's formulation, labeling text, company name, or product name without prior review by NSF, may result in removal of the product from the NSF WhiteBook™ listing.
