Formulation Revision Request Form

In order to more efficiently service those Standard 60 customers requesting formulation revisions, NSF International's NSF/ANSI Standard 60 Certification Program has created new standardized Formulation Revision Request Form.

Completed forms should be submitted to your assigned Program Representative for processing. They should be mailed in or faxed to (734) 913-5735. Upon receipt of a revision request, your NSF Representative will fill out the bottom of the form and return the form to you via fax indicating the expected completion date, and will note if the revision is authorized or if more information is required to complete the revision.

When submitting a revision request, please note the following:

  • Revision requests are facility specific. A revision for multiple facilities should be submitted on individual request forms (one per facility). A revision for multiple products at the same facility may be submitted on one form.
  • Failure to submit MSDS and composition information for a revision request requiring addition of suppliers or ingredients may significantly increase turnaround time for completion of request.
  • Customers requesting addition of alternate suppliers of the same authorized ingredients may begin implementation of the revision immediately after receiving the returned form with the bottom portion completed by NSF unless otherwise notified by NSF.
  • After further toxicological review, NSF reserves the right to request additional testing of the product before authorizing the revision request.
